Preventive Maintenance Agreement Template

A preventive maintenance agreement turns a one-off job into a predictable recurring relationship: it nails the visit cadence, the price terms, and how either party exits. Three things make this more than a single-trade sample. The auto-renewal toggle is on by default with a 30-day cancellation notice — turn it off and the term clause swaps to an explicit no-auto-renewal statement. The approval-dollar threshold means out-of-scope work over a set amount needs your customer's written approval first. And a written service report after each visit is built into the visit-schedule section, so the agreement has teeth.

Nothing here is computed. The flat fee, per-visit rate, member discount, emergency rate, approval threshold, late fee, and insurance amount are contract text — they are echoed into clauses, not run through a billing engine. Price your plan in recurring pricing or job-cost quote; the late-fee math lives in invoice late fee. Switch the pricing model to per-visit to bill by the visit instead of a flat term fee — only the active price merges into the clause.
