How this template works
This is a fill-in form, not a calculator — there is no formula and no computed result. Type into the fields on the left and the one-page intake on the right updates live; print it or save it as a PDF when you're ready.
Pick a trade preset (landscaping, cleaning, pest control, HVAC, handyman, or general) to label the service type and suggest a problem-description hint for that trade. The preset only relabels — it never changes which fields appear, so this single generator covers every trade. The Sample-filled toggle drops in a complete worked example; Blank clears it back to write-on lines so a fully blank sheet is a legitimate print-and-hand-fill artifact.
Every field is optional and nothing blocks printing. The core spine stays visible; secondary fields tuck into More fields so the form stays short — but the printed sheet always shows every section with a blank line for each. The service address is kept separate from the billing address because they often differ, and the Access & site-notes block (gate code, parking, pets, where the meter/panel/spigot is) is the field-service detail generic intake forms leave out.
The account reference is free text you type — it is never auto-generated, and nothing you enter is saved. There is no customer list, lead pipeline, or follow-up automation here.
