Pesticide Application Log Worksheet

This is a blank record sheet, not a calculator. One row is one application: you fill in the date and time, who the work was for, the treatment site, the product brand and EPA registration number, the rate and total amount, the purpose and target pest, the area treated and its unit, and — optionally — the weather and the restricted-entry interval (REI). It computes nothing: no dose, no conversion, no totals, no in-range check. The EPA registration number and license number are free text — nothing is validated or looked up.

The column set mirrors a typical state pesticide application form (Property owner name & address · Treatment site if different from owner · Date & time of application · Pesticide brand (label) name · EPA registration # · Rate of formulation per unit area · Total amount used · Purpose of application · Target site / pest), with weather and REI columns added for operator convenience. The Area column is numeric and the Area unit is an in-cell dropdown (sq ft · acre · linear ft) — they stay independent cells, so nothing is auto-converted.

For reference, a federal restricted-use pesticide record (USDA AMS / 7 CFR 110) usually captures:

• Brand name and EPA registration number of the pesticide applied
• Total amount of the pesticide applied
• Size of area treated
• Crop, commodity, stored product, or site to which the pesticide was applied
• Location of the application
• Month, day, and year of the application
• Name and certification (license) number of the certified applicator
• (If applicable) the supervising certified applicator for an application made by a non-certified applicator
• Keep records for 2 years from the date of the restricted-use application
